**Membership plan Details:

Does not include the following:

• No annual maximum, deductible, or waiting period
• No Preauthorization Required
• Enjoy Plan Benefits 365 days from Enrollment
• Plan must be paid in full at the time of enrollment
• 5% Savings on Annual Plan Renewal when paid in full on or before the anniversary date
• Membership savings can only be enjoyed when treatment is paid in full at the time of service
• Enrollment fee is non-refundable or transferrable to another patient
• Participation cannot be combined with any other offer or dental plan
• Membership plan is exclusive to Pearl Dental Studio and cannot be utilized in any other dental or specialist office
• Membership savings cannot be combined with any other discounts or third-party financing
• Membership is not applicable to any treatment needed following any type of venturing where a lawsuit and therefore outside medical care disability or Workmen's Comp. type insurances are involved
• Any unused benefits or procedures are not carried over to the following year
• If a patient is diagnosed with periodontal disease and has already paid for the Preventative Plan, the patient will be responsible for paying the difference to the Periodontal Plan to enjoy its benefits
• *Membership Discount for Reconstructive/Rehabilitation or Cosmetic Dentistry is 10%
• *The membership plan only covers dental services. Products are not included.
